Lewisham CEPN Darzi Fellow

The  Fellow will appraise evidence-based training for diabetes, in order that innovative training and education initiatives can be co-designed with relevant stakeholders (patients, primary care workforce, acute specialists, community care, voluntary care). Significant stakeholder engagement will be involved, including visits to all GP practices, at which their preferences for training delivery will be discussed in order that the most time-economical but effective delivery methods are commissioned

As well as the focus on diabtes in general, there will be an additional emphasis on training to support patients with the dual diagnoses of diabetes and dementia, and diabetes and cancer, and on diabetic foot complications. Baseline metrics will be captured for them and training to support personalised (and carer/family inclusive) care will be designed to enhance outcomes.

At the end of the Fellowship period, it is envisaged that standardised work-based competencies will be elaborated, and a training framework to support them, which will radically enhance primary and community care support for patients with diabetes. These competencies will align with, and complement, the national diabetes prevention programme. A study will also be undertaken into the viability of generalising the new training model across all LTCs and other priority conditions.

The outcomes envisaged are improved confidence and skills in the workforce, patient and clinician behaviour change in the management of diabetes, improved QOF performance for diabetes indicators, reduced referrals to secondary care as a result of more proactive and standardised management of diabetes and its associated conditions, and, ultimately, a much-reduced incidence of diabetes in Lewisham. Physical Activity Clinical Champions

A free, fun, interactive practically and clinical relevant session looking at the importance of physical activity for the health of the population of Lewisham and we’ll have information on local services and referral pathways.
